Early Years Quality of Life Questionnaire

Before completing the questionnaire we would like to ask you a few demographic questions.

This questionnaire asks you about different aspects of your life as a carer for your child. Please think about your experience within the last two weeks and select the response that best applies under each statement. There are no right or wrong answers; we are just interested in what life is like for you as a carer. The questionnaire shouldn’t take more than 15 minutes. Please answer all questions as honestly as you can.
