Hft Surrey supports people with learning disabilities to live with more independence and choice across the Surrey area, including Kingston, Sutton, Surbiton, Chessington, and New Malden.
Their person-centred approach supports people to take control of their lives. They work closely with the people they support, their families and carers, and health professionals to deliver services that are personalised to individual needs. Their support workers empower the people they support to make their own choices about how they live, using their innovative Fusion model of support to keep people at the centre of everything they do.
At Hft Surrey they support over 140 people with many different types of learning disability, and have particular experience in supporting people with autism and dementia. What’s consistent throughout, is their focus on ensuring that everyone is supported to live the best life, and achieve the best outcomes possible.
